SMART Support Consultation:Senior dog Form Your name, and the name of any other adults who may live in the home: Your dog's name: Your dog's, sex, age and breed/mix: Email * What would you like help with? Does your dog have any medical conditions? What medications is your dog currently on, and what is it treating? Have you noticed any recent change in behaviour? If so, what? Have you spoken to your veterinarian about the behavioural or physical challenges you are facing? If so, what did they say? How well do YOU sleep during the night? Great - My dog doesn't disturb me and I wake up rested, ready for a new day. Average - I hear my dog get up and either change position, or go drink some water. I'm able to go back to sleep quickly. Below average - I have to get up one or twice to let my dog go pee during the night or early morning. My sleep could be better. Poor - My dog has trouble settling in the evening. Lots of pacing, whining, potty breaks multiple times a night. My sleep quality is suffering, and it's affecting my day. My sleep is terrible - but it has NOTHING to do with my dog! How are YOU feeling right now? What support do YOU have available to you? Being the solo care giver to a senior dog can be very isolating, not to mention the physical and emotional toll. Do you have someone to help you? The anticipation of a loved one suffering, or approaching the end of life can have an enormous toll, especially if you're the primary care giver. Having a plan in place can help you prepare for those final days. If you would like help with this we can either chat in person during our call, OR if you would prefer I can send you the resources I have available, and I can support you via email. I already have a plan in place - this isn't something we need to cover in our call. Thank you so much for your discretion - I'm not emotionally able to talk about this in person, but I would love for you send me resources after our call. This is something I would love to chat about with you during our call. I understand that this can be an emotional topic, but it's also filled with love and appreciation for the wonderful life my dog has lived. Please give me any additional information that you think might be helpful. I understand that a SMART Support Consultation is designed to compliment the work I do with my veterinary Team. Saffrina can not diagnose medical conditions or comment on specific pharmaceutical medications. Check here Thank you! I’ll send you a Zoom link the day before our consultation - please reach out if you have any questions.
